How the Care Model Works
A four-domain, hybrid intervention combining in-home case management with structured environmental exposure designed for integration into existing care plans and value-based contracts.
OMOHC delivers a community-based behavioral activation program that addresses physical, mental, behavioral, and social health through a coordinated, multi-modal intervention. The model combines in-home case management and therapeutic coaching with structured, evidence-informed environmental exposure protocols translating nature-based and mindfulness-based practices into clinically credible, reimbursable programming.
The intervention is designed to reduce systemic and physical barriers to behavioral health access, particularly for populations with chronic disease, co-occurring behavioral health conditions, and social determinant risk factors. All programming integrates with existing PCP and behavioral health treatment plans this is adjunctive support, not standalone therapy.
Service delivery is hybrid: digital resources and remote coaching paired with community-based, in-person environmental exposure sessions. This structure enables deployment across plan populations while maintaining the clinical fidelity of in-person behavioral activation.
